The Alcotest is the breath testing instrument utilized in New Jersey.  If you get arrested for DUI in New Jersey you must submit breath samples.  When you operate a motor vehicle in New Jersey you give your implied consent to submit to a breath test.  If you refuse to give breath samples you will be charged with Refusal.  It is possible that the police will ask you to consent to a blood draw in certain circumstances or will obtain a warrant where a Judge will order you to submit to same.  Please note that you never need to provide a urine or hair follicle sample in New Jersey.  In the last column please make the entire 3rd offender box the same shade
